Contact your benefits provider to confirm that wellness foot care services are eligible under your plan.
Health Savings Account and Flexible Spending Account payments are welcomed when supported by a Letter of Medical Necessity (LMN) from your physician.
Self-payment will be required if a transaction is declined. Upon request, an itemized receipt can be provided for reimbursement submission.

• Post-fungal treatment
• Post-ingrown nail care
• Excessive callus management
